Plot Summary
- This scenario takes place in the respiratory ward of a hospital and follows the process of admitting a female patient from the emergency room. After confirming room availability, two nurses guide the patient to her room, conduct initial assessments, and gather necessary information, such as medical history, symptoms, and other relevant details from the patient and her caregiver. Key symptoms include persistent cough, shortness of breath, and chest discomfort.
- The nurses also evaluate the patient's pain and fall risk, ultimately providing fall-prevention guidelines. The caregiver is briefed on hospital policies, meal and shower arrangements, emergency procedures, and visiting hours. The nurses escort the patient and her caregiver to the room.
Nurse 2: Ms. Na, please change into these hospital clothes, and you may place your belongings in the locker over there. (Attaches name tags to the room and bed and sanitizes hands.)
Nurse 1: I’ll now need to ask a few questions for our records.
Caregiver: Since my mother has a severe cough and speaking is difficult, I’ll answer on her behalf.
Nurse 1: Of course. Please stay nearby and listen in as well, Ms. Na.
The nurses sit with the patient and caregiver to begin gathering information.
Nurse 2: Ms. Na, I’ll be putting on an ID bracelet for you now. It might feel a bit uncomfortable, but please don’t remove it. We’ll need it to confirm your identity for tests. Now, let’s check your blood pressure and temperature. (Takes vital signs.)

Nurse 1: Ms. Na, do you have any pain?
Patient: (Nods)
Nurse 1: On a scale from 0 to 10, where 0 is no pain and 10 is the worst pain, how would you rate it?
Patient: (Indicates a number with her fingers)
Nurse 1: Can you describe the pain? Is it a sharp stabbing pain, a dull pressing pain, or more of a burning sensation?
Patient: It feels like pressure, and it’s uncomfortable.
Nurse 1: How often does it hurt?
Patient: Constantly.
Nurse 1: What makes it worse?

The nurses explain the hospital rules and patient guidelines.
Nurse 2: Please bring your personal items like toiletries, slippers, tissues, and a water cup. You can purchase these at the convenience store on the first floor. Meals are provided according to a patient-specific diet. Visiting hours are at 10 AM each day, though it may vary. Press the call bell or dial the nurse’s station for assistance, and make sure to have your doctor’s approval for any outings.
Nurse 1: Ms. Na, because of your breathing difficulties, keeping the bed elevated may make you feel more comfortable. Please rest and feel assured that we’re here to help improve your health.
Caregiver: Thank you very much.
